Discharge Process
Planned Discharge
Patient completes the initial and actual management of the disease and the patient need doesn’t require direct supervision or active intervention from the hospital/ healthcare personal.
Steps of planned discharge
- Written order by doctor
- Information to other department (billing, insurance, dietary etc)
- Return of medicines / consumables to pharmacy
- Check payment of bills
- Discharge summery and other documentations
Unplanned Discharge
This includes LAMA (Leaving against medical advice), DAMA (discharge against medical advice) or Discharge at/on request.
Steps of unplanned discharge
- Information to other department (billing, insurance, dietary etc)
- Return of medicines / consumables to pharmacy
- Consent to be obtained from patient/ attendants
- Check payment of bills, Discharge summery and other documentations
Process
- Development of a care plan for post discharge care Treating Physician/Surgeon
- Arranging for the provision of services, including patient/family education and referrals. Ward in charge nurse/public relation
- The co-ordination related to discharge with other team members Unit head/Resident Doctor/Nurse in charge
- Preparation of Final discharge summary Resident Doctor
- Preparation of final bill.
